As required by the Ohio Revised
Code, the Scioto
County Board of MR/DD has established a system to report, investigate, review,
remedy, and analyze incidents that adversely affect
the health and safety of the individuals we serve and to monitor preventative
actions taken to ensure their health and safety. Definitions and
descriptions of such incidents are listed below, along with contact information
for reporting violations. The
Ohio Administrative Code Section 5123 is the source of all the following
information.
A Major Unusual
Incident (MUI) is the alleged, suspected, or actual occurrence of an incident
that adversely affects the health and safety of an individual, including acts
committed or allegedly committed by one service recipient against another
service recipient. Major unusual incidents include, but are not limited
to, the following:
- Abuse that is physical, sexual,
or verbal in nature.
- Misappropriation - depriving,
defrauding, or otherwise obtaining the real or personal property of an
individual by any means prohibited by the Ohio Revised Code.
- Neglect - failing to provide an
individual with any treatment, care, goods, supervision, or services
necessary to maintain the health and safety of the individual when there is
a duty to do so.
- The death of an individual.
- Any incident involving an
individual that requires the involvement of law enforcement and results in
the arrest of, filing charges against, or incarceration of the individual.
- Attempted suicide by the
individual, regardless of whether any harm resulted.
- Fire, natural disaster, or
mechanical failure at any place at which the individual receives services
that results in overnight relocation or an inability to provide the
individual with services for a minimum of 24 hours.
- An incident in which an
individual cannot be located for at least eight hours unless his/her plan
states a different period of time, prior arrangements have been made, or
other circumstances indicate that the individual is in immediate jeopardy.
- Medical emergency - the sudden
onset of a medical condition that requires emergency medical intervention.
- An unplanned or unscheduled
hospital admission.
- An injury of unknown or
suspicious origin that requires treatment that only a physician, physician
assistant, or nurse practitioner can provide.
- An injury of known or
non-suspicious origin that has a significant impact on the individual's
physical health.
- The use of any behavior support
method, including restraint or time-out, that is implemented in a manner
prohibited by the Ohio Department of MR/DD.
- Any violation of the rights
enumerated in Section 5123.62 of the Ohio Revised Code that adversely
affects the health or safety of an individual.
- A series of similar unusual
incidents that may have an impact on the health and safety of an individual.
An Unusual
Incident (UI) is an event or occurrence involving an individual that is not
consistent with routine operation, policies, procedures, or the care and
habilitation plan of the individual, but is not abuse, neglect, or a major
unusual incident as defined above.

suspicious acts or incidents, contact our Case Management secretary or MUI
Investigator at (740) 353-4677. If the call is made after hours, the
on-call person will respond and forward reports to the MUI Investigator as
appropriate. Incidents can also be reported to Ohio Department of MR/DD
through a 24-hour hotline at 1-866-313-MRDD or 1-866-313-6733.
